প্রাথমিকভাবে নির্বাচিত আবেদনকারীদেরকে আগামী ২৬ আগস্ট থেকে ১৫ সেপ্টেম্বর ২০২৫ তারিখের মধ্যে নিকটস্থ ডাচ-বাংলা ব্যাংক এর শাখায় (যেখানে শাখা নেই সেখানে ডাচ-বাংলা ব্যাংক মোবাইল ব্যাংকিং অফিসে) অবশ্যই পিতাসহ (পিতা মৃত হলে মাতা, পিতা ও মাতা উভয়ে মৃত হলে অন্য বৈধ অভিভাবক সহ) উপস্থিত হয়ে Summary Sheet (Primary Selection Letter) এর সাথে নিম্নলিখিত সংযোজনীসমূহ অবশ্যই দাখিল করতে হবে:
১. আবেদনকারীর সদ্য তোলা ২ কপি পাসপোর্ট সাইজের ছবি। ২. আবেদনকারীর পিতা ও মাতার সদ্য তোলা ১ কপি পাসপোর্ট সাইজের ছবি ও জাতীয় পরিচয়পত্রের ফটোকপি. ৩. এস.এস.সি পাশের ট্রান্সক্রিপ্ট/মার্কশীট এর ফটোকপি. ৪. এস. এস. সি. পাশের রেজিস্ট্রেশন কার্ড ও প্রশংসাপত্রের ফটোকপি.
৫. সর্বশেষ পাশকৃত শিক্ষা প্রতিষ্ঠানের প্রধান অথবা পিতা/মাতা চাকুরীরত হলে সংশ্লিষ্ট প্রতিষ্ঠানের উচ্চপদস্থ কর্মকর্তা অথবা অন্যান্য ক্ষেত্রে ১ম শ্রেণীর সরকারী কর্মকর্তা/ওয়ার্ড কমিশনার/ইউপি চেয়ারম্যান থেকে প্রত্যায়িত পিতা ও মাতা/অভিভাবকের পেশা উল্লেখপূর্বক আয় বিবরণীর মূলকপি । পিতা/মাতা মৃত হলে আয়ের বিবরণীতে তা অবশ্যই উল্লেখ করতে হবে. ৬. প্রাথমিক বা জুনিয়র পর্যায়ে বৃত্তি পেয়ে থাকলে প্রয়োজনীয় সনদ/প্রমাণপত্রের ফটোকপি. ৭. পিত/মাতা মৃত অথবা বিবাহ বিচ্ছেদ হলে/আবেদনকারীর ভরণ পোষণ প্রদান না করলে সে সংক্রান্ত সনদ/প্রমাণপত্রের ফটোকপি. ৮. আবেদনকারী শারীরিক প্রতিবন্ধী হলে প্রতিবন্ধী সনদ/প্রমানপত্রের ফটোকপি.
**বিশেষ দ্রষ্টব্য:
(i) উপরোক্ত সংযোজনীসমূহের মূলকপি প্রদর্শনের জন্য অবশ্যই সঙ্গে নিয়ে আসতে হবে (যদি কোন কাগজপত্রের মূলকপি বিদ্যালয়ে অথবা কলেজে জমা থাকে অথবা শিক্ষাবোর্ড হতে প্রাপ্ত না হলে সেসব কাগজের ফটোকপি/ অনলাইন কপি বিদ্যালয় অথবা কলেজ থেকে সত্যায়িত করে আনতে হবে). (ii) নির্ধারিত তারিখের মধ্যে ডাচ-বাংলা ব্যাংক এর যেকোনো শাখায় (যেখানে শাখা নেই সেখানে ডাচ-বাংলা ব্যাংক মোবাইল ব্যাংকিং অফিসে) উপস্থিত হয়ে উপরোক্ত সংযোজনীসমূহ দাখিল করতে ব্যর্থ হলে তার প্রাথমিক বাছাই বাতিল বলে গণ্য হবে. iii) প্রাথমিকভাবে নির্বাচিত আবেদনকারীদের যাবতীয় তথ্য ও সংযোজনীসমূহের সত্যতা যাচাই সাপেক্ষ্য নির্ণীত মেধা তালিকার ভিত্তিতে বৃত্তিপ্রাপ্তদের চূড়ান্ত তালিকা পরবর্তীতে প্রকাশ করা হবে.
